What ⓷ means
The double-circled 3 marks item three in a numbered list. A 1999 Japanese Standards Association proposal records double-circled digits one through eight as dentists’ markers for tooth positions. It proposed adding them to JIS X 0213, a Japanese character standard, to keep Japanese products compatible.
The same 1999 proposal says ordinary circled numbers and white numerals on black circles appeared in publications and newspapers.
Why can fonts draw ⓷ differently?
The font for ⓷ determines its rings, spacing, and numeral shape. A font without the glyph may show a substitute or missing-character box.

Questions people ask
1Why does ⓷ show as a box on some devices?
Because that device's fonts have no glyph for it. The character is still correct and still copies correctly — the box is a missing glyph, not a broken character.
