What 병 means
By itself, 병 has no single meaning; the whole word decides.
The same syllable occurs in words with different Hanja roots: 病 in the medical word 병, 甁 in 병맥주 (“bottled beer”), and 兵 in 병사 (“soldier”). These characters explain particular words; they do not give 병 a single meaning on its own.
병아리 means “chick” in native Korean, so a Hanja character is not behind every word containing this syllable. Read the whole word to tell which meaning fits.
How did Hangul—and this syllable—take shape?
Hangul was designed to write Korean speech more accessibly.
Before Hangul, Korean writers used systems such as Idu, Gugyeol, and Hyangchal, which adapted Chinese characters to record Korean. Hangul was created in 1443; Hunminjeongeum, a guide explaining the new script, was published in 1446.
The 1464 Sangwonsa Jungchang Gwonseonmun is identified as the earliest surviving Hangul manuscript. That dates surviving writing in Hangul, not the first use of 병.
Is 병 insulting when someone sends it alone?
No; by itself, 병 is not an insult.
The full compound 병신 can be derogatory when used about a person. That warning does not carry over to the isolated syllable, which also occurs in ordinary medical, bottle, military, and native Korean words. If the message contains only 병, ask for the surrounding word before settling on a translation.
Neutral as an isolated Hangul syllable; the complete Korean word sets its register.
Treat 병신 as derogatory when used about a person; do not apply that reading to lone 병.

1Why does 병 show as a box on some devices?
Because that device's fonts have no glyph for it. The character is still correct and still copies correctly — the box is a missing glyph, not a broken character.
