What ≢ means
It denies whichever triple-bar relation the formula uses.
That relation can mean identity, equivalence, or congruence. In modular arithmetic, congruent numbers leave the same remainder when divided by a fixed number; ≢ says the remainders differ.
Triple-bar notation developed alongside formal equality notation. The modern equal sign appeared in print in 1557; Gauss used a triple bar for modular congruence from 1801.
Formal mathematical notation; use it in a formula or explain the convention in prose.
Use ≠ for ordinary inequality; ≢ rejects a field-specific relation marked by a triple bar.

1Why does ≢ show as a box on some devices?
Because that device's fonts have no glyph for it. The character is still correct and still copies correctly — the box is a missing glyph, not a broken character.

A visible symbol can contain several code points. JS .length counts UTF-16 units; UTF-8 counts bytes.
