What ⒙ means
It labels the eighteenth item in a numbered sequence.
Unicode describes the full-stop, circled, and parenthesized numerals in this block as bullets for ordered lists; most were encoded for compatibility with East Asian character sets. A June 1992 character-set reference already lists the eighteenth full-stop form.
What is the difference between ⒙ and ⑱?
⒙ ends in a full stop; ⑱ encloses 18 in a circle.
⒅ is the parenthesized sibling, with the same list role but different punctuation.
Compatibility normalization can reduce ⒙ to 18.. The number and period remain, but the marker's special form is lost.

Questions people ask
1Why does ⒙ show as a box on some devices?
Because that device's fonts have no glyph for it. The character is still correct and still copies correctly — the box is a missing glyph, not a broken character.
