What 痼 means
It means a chronic illness or something deeply entrenched. The early Chinese dictionary Shuowen Jiezi records the older form 㽽 for a long-lasting illness; later dictionaries use 痼.
疒 is the illness component, while 固 supplies the sound. From its long-illness sense, 痼 also comes to describe something deeply rooted and hard to shake.
Why can 痼 describe habits?
It extends “hard to cure” from illness to behavior. 痼疾 names a chronic illness; 痼習 and 痼癖 apply the same idea to a stubborn habit or fixation. In Japanese, 痼疾 also names a long-standing illness and is read こしつ, roughly “ko-shitsu”.
Formal and literary; 痼 usually appears in compounds for chronic illness or entrenched habits, not as casual standalone chat.
痼 does not mean just any illness; persistence or entrenchment is the point.

1Why does 痼 show as a box on some devices?
Because that device's fonts have no glyph for it. The character is still correct and still copies correctly — the box is a missing glyph, not a broken character.
