What 皮 means
It means skin, hide, or an outer covering. In Chinese, the meaning extends to materials such as bark, casings, and thin sheets. Japanese and Korean words also use 皮 for skin; Japanese 皮肉, though, means irony or sarcasm as a whole word.
Two Shang-era oracle-bone examples—inscriptions on bones used for divination—from Huayuanzhuang East use the graph as a place name, not clearly as the word for skin. A later Chinese dictionary, Shuowen, explains the form as a hand removing animal hide. That traditional analysis links stripping to the noun “skin” or “hide”; it does not prove what the earliest inscription was meant to depict.
Is 皮 an insult?
It can be teasing or insulting, depending on the Chinese phrase. Chinese dictionaries record both playful naughtiness and a thick-skinned or shameless person, so calling a person 皮 is more pointed than describing a covering.
In 皮一下, 皮 gives the phrase a playful, mock-naughty reading. It can frame the sender’s own joke rather than accuse you.
A bare 皮 does not settle the sender’s intent. If it is unclear, check the surrounding words or ask what they mean; the same graph also appears in literal words and in Japanese or Korean compounds.
In Chinese, 皮 can describe a covering or turn personal and pointed when applied to someone.
Bare 皮 can suggest shamelessness; with an unfamiliar contact, do not assume it is affectionate.

1Why does 皮 show as a box on some devices?
Because that device's fonts have no glyph for it. The character is still correct and still copies correctly — the box is a missing glyph, not a broken character.
