What 頇 means
“Large-faced” is the old gloss; 顢頇 means muddled or careless.
A 2024 Taiwan Ministry of Education dictionary entry notes that 頇 is absent from 說文, an early Chinese dictionary. Its earliest dictionary record is in 玉篇, compiled in 543. The Song-era dictionary 廣韻 glosses 顓頇 as “large-faced.” The old face description later shifted into a criticism of how someone handled things.
In 顢頇, it is bluntly negative and rarely sounds like a neutral description.
Applied to someone's work, it can sound like calling them incompetent.
