The whole word carries the meaning: 什麼 is “what,” 怎麼 “how,” while 多麼 adds degree (“how” or “so”).
Later additions to 說文解字 (an early character dictionary) gloss 麼 as “small/fine” and analyze it as 幺 plus 麻 (the sound part); this does not date its first use.
