What 🟃 means
Decoration: a bullet, a divider or a flourish. The "pinwheel" in its name describes the shape: each of the three points is offset to one side, so the star looks as if it is spinning. "Medium" is its weight in a set of the same star drawn at different thicknesses.
It belongs to a family of geometric stars added to Unicode so that ornaments from Microsoft's Wingdings symbol fonts of the early 1990s could survive as real text. Before that, such stars existed only inside one font; change the font and the star turned into a random letter. Now it is a character of its own that any font may draw.
Neutral and decorative; it reads as typography, not emotion.
If it arrives alone, it is almost certainly decoration, not a coded message.

3Questions people ask
2Why do I see an empty box instead of 🟃?
Your font has no drawing for it. It is a rare symbol; installing a broad symbol font fixes it.
Is 🟃 an emoji?
No. It is a plain text symbol with no emoji version, so it appears in black and white in your font's style.

fsymbols draws rare signs with its own fonts, so they show here even where your device has none. An app or website without such a font shows an empty box (□) or a question mark instead. Turn our fonts off to see each sign as your device draws it by itself: a box here means a box in your chats and documents too.
Codes and encoding1 code points
A visible symbol can contain several code points. JS .length counts UTF-16 units; UTF-8 counts bytes.
