The two slang senses come from different places. Calling a person or a remark “cold” or “cold-blooded” has long meant unfeeling — that's the insult side. The praise side comes from sports talk: a player with “ice in their veins” stays calm when the game is on the line. The blue face can carry either, so the surrounding message does the work.
